Because "enter" can become a 2-byte instruction, it seems best to
solve it somewhere else.  There's an unused bit in the threaded code
which can be used to distinguish between a primitive and a high level

Alternatively, use an indirect threaded Forth, but that requires all
primitives to be wrapped.

What about placing the address of NEXT on the return stack?
