Asterisk behind firewall

So, config:
externip=<EXTERN-IP>
localnet=192.168.6.0/255.255.255.0
nat=yes

The problem is that I'm trying to perform a call from within my
network.  To any host this looks like the router's IP address.

However, if asterisk is trying to send to this address from the
_inside_ of the firewall it won't work!
